Forms and Documentation

Make sure you’ve read, completed and submitted all required forms and documents prior to your arrival on campus. Planning ahead will make your arrival on campus as smooth as possible. Note: All forms and documentation must be read and responded to prior to your arrival on campus for the program. You cannot attend classes until all required materials have been signed and submitted.

Due Dates:

  • MONDAY, MAY 1, 2017: Health Services Form Due
  • FRIDAY, JUNE 9, 2017: All other forms due to their respective offices

    NOTE: The Health Services Form requires extensive student health information, a physician’s authorization and signature. Please review this information immediately and plan accordingly in order to return the form in time. Deadline: May 1, 2017
  • Student Emergency Medical Information Form
    Download Student Emergency Medical Information Form (for RISD Public Safety’s use in case of emergencies; send to the RISD Public Safety Office) Deadline: June 9, 2017
  • Student Handbook
    Download Summer Programs Student Handbook
    Contains detailed information about arrival and departure; housing, dining and safety; academic and disciplinary policies and procedures. Please read the handbook carefully. Students should be completely familiar with RISD’s official policies and procedures contained in the handbook, prior to arrival on campus. Since the handbook contains a lot of important information, we recommend printing the pdf and keeping it nearby as a reference during the program.
  • Health Insurance Requirements
    Review Health Insurance Requirements
    The College requires all students to carry adequate medical insurance to help cover the extra expenses of medical treatment that is not covered by our Health Services. All medical insurance policies must be from a U.S. domestic insurance company. Medical insurance is mandatory for all students at RISD, including summer students. If you do not have medical insurance from a U.S. domestic insurance company, you may purchase a six(6)-week short-term medical insurance policy for $156. To assist in providing you with the coverage you will need during your summer studies at RISD, we have contracted with University Health Plans Insurance Program to offer the Summer Studies Student Health Insurance Plan. The Plan includes a local and national network of preferred providers, and is designed to be an affordable option.
